Sponsorship Form

Gif Aid adds an extra 25% onto every donation, therefore, it is important that we can claim every donation that has been ticked. You can email a photograph of your sponsorship form to [email protected] or post the sponsorship to Joseph’s Smile, 206 Cwmgarw Road, Brynaman, Amanford, Carmarthenshire, SA18 1DH.
