Thank you for joining the Wales vs England charity football match and helping our great cause. We are delighted that you have chosen to help our charities and the children we support. This game will not only help raise essential awareness of childhood cancer, but it will also help children access lifesaving or lifechanging treatment that’s currently unavailable to them on the NHS. Venue: Swansea.com stadium, SA1 2FA

Pre-Match Training: All players and managers will arrive at Swansea.com stadium for pre-match training with their managers at 12pm, light lunch included.

Pre-Match Entertainment: Stadium entertainment will commence at 2:45pm with National Anthems being performed prior to kick off, with full teams in attendance.

Football Match: Kick off will commence at 3:30pm. Game duration will be for 90 minutes, with added half time.

Post Match Hospitality: Complimentary player’s presentation and hospitality package included for all players and sponsors after the game.

Accommodation: Please state below if you require accommodation in Swansea which will be at Morgan’s Hotel, SA1 1RR or at the Marriott Hotel, SA1 3SS.

The player/manager consents to participate in a charity football match on Saturday 11th May 2024 at Swansea.com stadium. In agreeing to this statement, the player/manager is not under any contractual obligation to attend, however, we do ask for notice if an emergency work commitment arises which results in the player/manager being unable to participate. By agreeing to this statement, I give full authorisation for the organisers to use a photo of the player/manager in promotional material related to this game only. As in all charity football matches, please note that player-on-player accidental injuries are not covered by our public liability. Media Consent *

We (Joseph’s Smile) are delighted that the BBC have commissioned Wales based independent television company Rondo Media to film us preparing for this charity event as well as the wider charitable activities. Rondo have requested that you are aware of their filming presence at the football match on the day, where they will be keen to capture the fun of the day with the players’ interaction and players / public interaction to reflect the support for the work of our charities. Rondo have asked that you confirm that you [or player’s name] consent to be filmed during the event, which may include recording incidentals such as pre and post match interviews, behind the scenes activity and possibly clip a small microphone onto their shirts during the match to capture the action. This will be in addition to general “news and publicity” coverage on the day. I should be grateful therefore if you could confirm that Rondo Media are granted the right to film [player’s name], including any image rights, for the use in all media worldwide in perpetuity.
